\u201dStandardization of laparoscopic surgery for patients with cancer of the colon and rectum in Health South East RHF – LapcoNor\u201d<\/span><\/p>\n October 2017<\/strong><\/p>\n LapcoNor is a regional project in Health South East, which aims to offer structured and standardized education in laparoscopic surgery for colorectal cancer to all gastro-surgeons in the region treating these patients. Both survivors and LIS physicians can sign up. The project is run in close collaboration with Lapco, the English national education program for laparoscopic colorectal surgery. Hence the name LapcoNor, with its associated logo.<\/p>\n The project was established by HS\u00d8 Spring 2014 in cooperation with the Education Center at OUS Ullev\u00e5l. Project owner is Ole Tjomsland, Director of Quality and Trade, Health South East. We have an established project group consisting of 10 surgeons and three surgery nurses from the major hospitals in the region. In addition, we have a resource group of key professionals from all health regions.<\/p>\n Background for the project<\/strong><\/p>\n Randomized controlled studies and meta-analyzes have shown that laparoscopic resection provides faster postoperative mobilization and fewer complications with the same oncological long-term outcomes as open surgery, both for colon and rectal cancer.<\/p>\n Good preoperative investigation and assessment, as well as surgical experience and technique are of crucial importance for good results. Many centers that have gone from open to laparoscopic surgery for patients with colorectal cancer, also in HSE RHF, have recorded increased complication rates in the transition phase from open to laparoscopic surgery, with increased occurrence of anastomosis leakage, a complication with significant morbidity and mortality.<\/p>\n Purpose of the project<\/strong><\/p>\n Currently, laparoscopy rates for colorectal cancer vary widely in the region, and the training in laparoscopic colorectal surgery is also different from place to place. The main objective of the project is to increase laparoscopy rates for the treatment of colon and colon cancer. This is done by providing standardized training by template from the English national education program Lapco. Evaluation of the project<\/strong><\/p>\n All hospitals in HS\u00d8 report their respective patient data (NPR data) to Dr. Foster’s Global Comparators, an international database where hospitals from all over the world submit corresponding data for analysis. This will be used to compare laparoscopy rates and treatment results before and after the implementation of our education program.<\/p>\n All learning operations (“hands-on” interventions) will be evaluated by standardized template and registered online in their own database in order to follow the development of the individual surgeon during training over time, and compare with the other surgeons associated with the training program. Clinical data will also be recorded and evaluated.<\/p>\n What happened?<\/strong><\/p>\n As mentioned above, we cooperate closely with Lapco International – England, and after their recommendation, they have started training trainers in laparoscopic colorectal surgery. The project team has completed the Lapco TT (Train the Trainer) course in Basingstoke 25th-27th. March 2015. This is a tailor-made interactive 2-day instructor course in laparoscopic colorectal surgery with specific educational objectives. The concept is adapted to Norway by template from Lapco – England and we have completed 9 courses with 6 participants (a total of 54 participants) during the period June 2015 – September 2017. The courses have been held in English in English in 2016 with solid support from experienced English instructors. We have now been evaluated by Lapco as independent enough to continue the courses in Norwegian. The last 4 courses were therefore held in Norwegian.<\/p>\n What happens next?<\/strong><\/p>\n An additional “Training Coach – LapcoNor Course” will be arranged in 2017, where hospitals without representatives in the HS\u00d8 project group and hospitals from the other health regions will be prioritized.<\/p>\n The project has started structured training of surgeons from 01.01.16. using standardized learning methods conveyed through our Training Trainer course. These are used in “hands-on” operations evaluated by standardized template. From winter 2017\/18 we will have the option of online registration of all these learning operations in a separate database that will be used for research on the results of the training program.<\/p>\n Lapconor held its first first introductory course for surgeons wishing to participate in the individual training in laparoscopic surgery for colorectal cancer on the 9th-10th. February and 19-20. October 2017 at the pathological department Ullev\u00e5l. <\/b>The courses were held over two days, had 16\/15 participants and 4\/4 observers. The theoretical part included the review of oncology treatment principles for colorectal cancer and pedagogic principles for the structured, standardized training of the individual surgeon. Practical exercises in standardized operating techniques were performed on simulator models with similar operations.
